Community Pharmacy Future results publlished
With the initial evaluation of the Pharmacy Care Plan service completed (see below), an article has been published by the peer-reviewed Clinical Pharmacist. The article outlines the success from the first Community Pharmacy Future services, touches on the Pharmacy Care Plan service and highlights how these service evaluations could be used to evidence the role of community pharmacy to improve patient outcomes and deliver efficiencies for the NHS.

The first research paper for the Pharmacy Care Plan service has been peer-reviewed and published by PLOS ONE. The research paper was written in collaboration with independent academics from the University of East Anglia.

Community Pharmacy Future project in the news
The Community Pharmacy Future project continues to attract interest from across pharmacy and the NHS

The Pharmaceutical Services Negotiating Committee (PSNC) has produced a “five-point forward plan” which outlines the services it would like to see the NHS commission through the national Community Pharmacy Contractual Framework for England. 
Two of the five points focus on supporting patients with long-term conditions, especially COPD and older patients on multiple medicines which are based on the results from the first phase of the CPF project.

Field leaders meeting supports six-month reviews
Today (16 September 2015), the CPF project team met in Wakefield with field team leaders for the pharmacies conducting the Pharmacy Care Plan service.  After an update of initial findings from the recruitment phase of the project, the group discussed how best to support pharmacy teams delivering the service, to ensure that six-month reviews, due to start this month, are as beneficial as possible for our patients.

Pharmacists' focus group gives its views
Today (1 October 2015), a focus group of pharmacists who are delivering the Pharmacy Care Plan service was held in Yorkshire to allow the CPF service design team to hear directly about the practicalities and impacts of service.

Recruitment completed for Pharmacy Care Plan service
More than 860 patients have been enrolled in to the Pharmacy Care Plan service as the recruitment phase ends, well within the target range of 800-1,000 patients set for the project.

As of 26 June, 865 patients had been enrolled for the Pharmacy Care Plan service during the 18-week launch and recruitment phase. Of these, 705 have already had their baseline review with their pharmacist and this data has been entered on PharmOutcomes. An initial view of the data shows that a very broad range of patients has been recruited, with a wide spread of age ranges, medical conditions and patient activation measures.

The Community Pharmacy Future (CPF) project has started to recruit patients as part of the second phase of its work in developing new pharmacy services. Patients using 51 pharmacies in the North Kirklees and Wakefield areas of West Yorkshire are being offered the Pharmacy Care Plan service

Using community pharmacies to identify undiagnosed cases of chronic obstructive pulmonary disease (COPD) at an early stage would provide “significant NHS and societal benefits” and save the NHS more money than the service costs to deliver, a new study shows.

New data highlights how community pharmacy can play a bigger role in the delivery of patient care.

The delivery of healthcare services by pharmacy could lead to a cost saving for the NHS of over £470 million per annum if they were rolled out to 11,100 pharmacies across England, according to new research.

FOMM Support Service - Celebrating success with the Four or More Service
Your fantastic success in providing the Four or More Medicines Support Service was celebrated this month. Pharmacists and staff from all four companies shared their experiences of delivering the service and the benefits it is bringing to them and their patients.
We heard about the obvious success of managing to recruit and retain patients for the service, and the clear benefits being seen by them as a result. But two other successes also became clear during the event.
